Waiver and Liability Release<br/>

In consideration of the opportunity to participate and/or my participation in the Bosco LAN Party<br/>
 <br/>
 <br/>
I agree as follows:<br/>
1. If I am responsible for any damage to another person(s) or organization or their property either physically or <br/>
mentally, I agree to compensate the situation appropriately.  Compensation may include, but is not limited to, <br/>
financial reimbursement of damaged property, expulsion from current and all future events, and a formal apology. <br/>
 <br/>
2. The equipment I bring will be the sole responsibility of myself and I acknowledge that none of the hosts, <br/>
administrators, Saint John Bosco Parish, St John Bosco College, and any affiliates are<br/>
accountable for any lost, stolen or damaged goods. I am aware that the Bosco LAN Pary involves the risk <br/>
of serious damage to any equipment brought to the activity including damage from electrical malfunction, fire, <br/>
flood and acts of God and that not all hazards or risks arising out of, connected with, or in any way associated <br/>
with my participation in this activity can be foreseen and abolish the hosts, administrators, Saint John Bosco Parish,<br />
St John Bosco College, and any affiliates of any responsibility arising from damage to any equipment brought to the event. <br/>

3. I am voluntarily participating in this activity with knowledge of the <br/>
dangers involved and agree to voluntarily assume the risks of any and all liability arising out of, connected with, <br/>
or in any way associated with my participation in this activity.<br/>
<br/>
If I am under 18, then my parent/guardian has authorized me to participate in this activity, and also <br/>
agrees to assume the risks of any and all liability arising out of, connected with, or in any way <br/>
associated with my participation in this activity. Furthermore, by registering, my parent/guardian <br/>
understands that I may be exposed to, or playing games with mature themes, <br/>
and authorizes my involvement in those games.<br/>

4. My parent/guardian and myself, our heirs, executors, administrators and assigns, waive and relinquish any <br/>
and all claims, demands, actions or causes of action which I may have against the event hosts, event sponsors, <br/>
its staff, representatives, or volunteers and any of their predecessors, successors, heirs, affiliates or <br/>
assigns (&#8220;Released Parties&#8221;), for any injuries, damages, liability, expenses or losses of any kind to my person <br/>
or property arising out of, connected with, or in any way associated with my participation in this activity. I also <br/>
agree to indemnify and hold harmless the Released Parties for any and all claims, demands, actions or causes <br/>
of action, injuries, damages, liability, expenses or losses of any kind arising out of, connected with, or in any <br/>
way associated with my participation in this activity, including but not limited to any attorney fees which I incur <br/>
as a result of any of the foregoing. <br/>

I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND ITS CONTENT. I AM AWARE THAT <br/>
THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND THE RELEASED PARTIES. I <br/>
HAVE AGREED TO IT OF MY OWN FREE WILL. I FURTHER ACKNOWLEDGE THAT NO MODIFICATIONS TO <br/>
THIS DOCUMENT WILL HAVE ANY EFFECT ON ITS ORIGINAL CONTENT.<br/>
